Instructions for completing the BRAIN 2010 Online Application Form (overview)
- Read all of these instructions before proceeding
- Download and read through the Application Preview Scroll down through the PDF to read all the pages.
- If, after reading through the Application Preview you decide to apply, then obtain a "My CBN" username and password (see below for detailed instructions)
- Log in to My CBN Web Portal using your new account (see below for detailed instructions)
- Change your My CBN Password, then log out
- Log in again to My CBN Web Portal using your changed password
- Select the Online Application Form
- Read the instructions presented on the first tab of the Online Application
- Fill in the requested information on all sections
- Exit the Online Application
- Be certain to also fulfill all other application requirements
Obtain a My CBN Username and Password
- The deadline has passed for 2010; no new accounts will be created
- Instructions below are retained for reference; no new accounts will be created
- Go to the Login Page for My CBN; the link is in the last item in this list
- Log in as "Guest"; select the Guest Account button, and do not enter any username or password; click "Login"
- Select the web portal application link "BRAIN_Applicants_Start_Here" (NOT Account_Request)
- Read the instructions presented in the application, then click the Start button
- Fill in the requested information, then click the Submit button
- Read the closing, then click the End button
- Account Requests made before 5:00 PM Monday through Thursday will be processed within one business day, Tuesday through Friday. Requests made Friday through Monday will be processed on Tuesday.
- Georgia State University is closed from December 21st, 2009 through January 4th, 2010; expect delays in the processing of Account Requests during this holiday period.
- You will receive your My CBN Account Username and Password by email within one business day after processing.

After you receive your My CBN Username and Password, Login and change your Password
- Go to the Login Page for My CBN; the link is in the last item in this list
- Enter Your My CBN Username and Password, then click Login
- Click the web portal link to Change your My CBN Password
- Your browser will redirect to a page with instructions and links. Follow the directions found there.
- Enter your My CBN Username in the Login ID field
- Enter your originally assigned My CBN Password in the Old Password field
- Enter a new password; use at least 6 mixed alphanumeric characters, please
- Re-enter the same new password in the Verify field, then click Change Password
- Use your browser's Back button to return to the My CBN Login Page

Complete the BRAIN Online Application Form
- The deadline for online application has passed
- Instructions below are retained for reference
- Go to the Login Page for My CBN; the link is in the last item in this list
- Enter Your changed My CBN Username and Password, then click Login
- Click the web portal link to BRAIN
- Read the instructions presented on the first tabbed section of the Application Form
- Fill out the requested information on all tabbed sections of the Application Form, as instructed
- When finished, click the "Finished? Click Here" button; your information will be saved, and you will be logged out

Application Requirements
- Completed BRAIN Online Application Form
- An official copy of your most current undergraduate transcript.
- Resume
- Two letters of recommendation (at least one from a science instructor or advisor).
- Mail all printed materials to: Neuroscience Institute, Georgia State University, c/o Veronica Williamson, PO Box 5030, Atlanta, GA 30302-5030
